Looking for an easy grab-and-go resource that will guide you in different levels of prompting support while efficiently tracking data on a day to day basis?
The Levels of Prompting Monitoring Worksheet lets you document the following data each day:
1. Date
2. Activity (to help special education staff determine preferred versus non-preferred tasks)
3. Strategies (direct models, choices, hand over hand, etc.)
4. Levels of Prompting (independence, verbal, visual, physical)
On your first day, start with the checkbox for independent. If the child is not able to do a task independently, write an X in the box and move to the next strategy below it. Continue to write X in the boxes until the student is successful and write a checkmark on the box. The following day, start at the level of support the child was successful at. Work you way up or down the prompting hierarchy each day.
To print this worksheet, highlight the worksheet and select print "selected cells."
