This is a great sheet on the first day(s) of school. It will allow you to better understand your new students' background, strengths and weaknesses, and goals for the year. It also allows you to learn a little more about your new students personal interests.
I've used it as their first HW assignment. In class the next day, I collect them and anonymously answer each student's question (the last question on page 2). It's lots of fun!
