Description

Research shows that positive teacher-student relationships improve student learning - especially during stressful times. Keeping in touch with your students' mental health and wellness is extremely important as an educator. This all falls under Social Emotional Learning (SEL). SEL focuses on developing self-awareness, self-control, and interpersonal skills vital for success in life. We have created 30 unique digital Wellness Checks in Google Forms that are perfect to get a daily glimpse into how your students are doing while teaching to the whole learner.

Each of the 30 Wellness Checks in set 1 are unique, and they are great resources for classroom teachers, librarians, or an interventionist looking for a way to keep up on their students' mental health and wellness. With our Wellness Checks, you can use a different one daily, or give students the choice of which form to complete. Some of the forms can be posted in your LMS (like Google Classroom or Canvas) as a permanent form that students can complete at any time of the day. For example, our 'Wish List" Wellness Check could be filled out if students are in an emotional crisis.

These editable Wellness Checks are perfect to use at the beginning or end of each class, school day, school week as an SEL check-in for any classroom environment, online, blended, or face-to-face. The forms can be taken on any device (laptop, tablet, or smartphone). If needed, they could be printed as a PDF; however, they were not originally designed for that purpose. If you are interested in a set of them as a PDF, please email us to let us know, we will add it to our list.

What is Social Emotional Learning?

CASEL states that SEL 'is the process through which children and adults understand and manage emotions, set and achieve positive goals, feel and show empathy for others, establish and maintain positive relationships, and make responsible decisions." CASEL has five core competencies: 1) self awareness, 2) self management, 3) social awareness, 4) relationship skills, and 5) responsible decision-making. Our Wellness Checks help meet the self-awareness competency as students learn how to identify how they feel. This daily check-in helps them recognize different emotions including the intensity of those emotions.

What happens when a student's response is not what you expected?

It is vital to think through your procedures ahead of time to ensure you are prepared for ANY response from your students. You should research the resources your district has surrounding SEL and who to turn to in your district if you get an unexpected response. Ask yourself what will you do if a student says they are not doing well? Who will you refer them to if they are in distress? How will you track your referral to ensure the student gets the support? Keep in mind, some students may say things just to seek attention, so you must come up with a method to limit those false positives.

New to Google Forms or unsure what to do once you are done?

This YouTube Video from Simpletivity has a great overview on Google Forms. If you are interested in just how to implement our Forms with no modifications, you can skip to the 11:40 second mark to learn how to share out the form to students or whomever you want filling it out. In addition, the video also goes over how to view the responses to your form. If you have any questions about how to share the form with students or how to view responses, please do not hesitate to email us for some help!
